Personal Care Services: Bathing, dressing, grooming, toileting, walking, transfers, feeding, medication assistance, and routine skin care; estimated 2,000 hours annually.
Service Coverage: Ability to serve clients anywhere in Wayne County and provide substitute workers when regular staff are unavailable.
Qualifications: Provider must directly furnish services, meet applicable staff screening and training requirements, and hold required New York State registrations and licensing.
Term: Three years beginning in 2027, with two optional one-year extensions.
