Term: July 1, 2027–June 30, 2030, with one optional three-year renewal.
Caseload: 500 or 505 active cases per provider, with capacity for 575 or 580 clients, respectively, at no additional cost.
Services: Assessments, service plans, case management, referrals, benefits assistance, housing stabilization, and financial and legal advocacy coordination for adults 18+ with mental or physical impairments.
Response and visits: Initial visits within 24 hours for life-threatening situations or three working days otherwise; assessments within 60 days and monthly home visits for active cases.
Staffing: Average 30 cases per case manager, one supervisor per five case managers, a dedicated program director, and 24-hour emergency coverage.
Administration: Secure case records, timely case notes, monthly reporting, and management of an emergency client fund.
Experience: At least three years of relevant experience within the past five years.
