Staffing: Full-time program director, qualified case manager/social worker, and an experienced healthcare worker on-site at least 14 hours over two or more days weekly.
Program requirements: Benefits assistance, assessments, care planning, referrals, health screenings, wellness activities, and two elder abuse prevention sessions annually.
Facilities and access: On-site main office, accessible group activity space on-site or within half a mile, and language assistance appropriate to residents.
Service period: July 1, 2027–June 30, 2030. Please provide annual pricing, proposed staffing, service capacity, and the boroughs you can serve.
