What the job is
Services: Activity checks, indoor/outdoor video surveillance, background checks, and legally obtained records research to assess disability-related activities and unreported employment.
Staffing: One investigator and one vehicle per assignment unless otherwise approved; no subcontracting. Personnel must be physically located within each quoted territory and cover its entire area.
Evidence: Supply recording equipment and deliver authentic surveillance footage on computer discs with documented chain of custody; include media costs in the hourly rate.
Reporting: Initial contact within three working days, preliminary surveillance update within 24 hours, and signed narrative report within 15 days of referral.
Testimony: Hearing, trial, or deposition testimony and related preparation; testimony billed at the surveillance hourly rate.
Quote: Provide service rates, a separate hourly travel rate, territories covered, and confirmation of at least five years of Pennsylvania workers’ compensation investigation experience.
