Care: Weekly visits to nursing facility residents, including bedbound patients; one annual examination and cleaning per resident, plus additional treatments as needed.
Coverage: Provide 24-hour emergency services and a backup dentist for extended absences. Regular visits occur Monday–Friday, 8:30 a.m.–4:00 p.m.
Staff and equipment: Supply all dental staff, equipment, supplies, and sterilization.
Qualifications: Current Pennsylvania dental license, at least five years of dental experience, and a provider location within 20 miles of the facility. Services cannot be subcontracted.
Term and pricing: Three years with two optional one-year renewals. Please itemize examination, cleaning, and additional treatment rates.
Billing: Handle applicable Medicare, Medicaid, and third-party insurance billing and accept insurer UCR rates.
