Services: Comprehensive and limited exams, consultations, radiographs, preventive care, scaling and root planing, extractions, periodontal procedures, crowns, crown lengthening, biopsies, dentures, pathology evaluations, and pre- and post-surgical care.
Service setting: Off-site care within 25 miles of the center, plus on-campus evaluations and treatment as needed.
Schedule: As needed through August 31, 2027; regular hours Monday–Friday, 8 a.m.–5 p.m. Central Time. Respond to routine calls within four hours and emergencies on-site within one hour.
Qualifications: Dentist and assigned personnel with at least five years of relevant experience; current Texas dental licenses, dentist annual registration, DEA registration, and CPR certification.
Pricing: Please provide separate hourly rates for regular and outside regular hours, including personnel, equipment, supplies, transportation, and equipment cleaning, sterilization, and maintenance.
