Care: Services for adults aged 55 and older with physical or mental impairments, including dementia; minimum four hours at the center per service day.
Qualifications: Existing Maryland-licensed medical adult day care center with at least three years of experience; program director, registered nurse, activities coordinator, licensed medical director, and licensed social worker.
Transportation: Round-trip transportation for participants requesting it, with a rest-stop opportunity whenever transit exceeds one hour.
Volume: Historical monthly service days by area: Anne Arundel 141; Frederick 121; Howard 135; Montgomery 20910 90 and 20832 127; Prince George’s 163; St. Mary’s 96; Wicomico 144. Volumes are not guaranteed.
Pricing and term: Provide an all-inclusive daily rate, including transportation, for each area served and each of three base years plus two optional years, beginning April 1, 2027. Participants pay income-based fees, and supplemental funding may be needed to cover the full cost of care.
