What the job is
Service model: Parents as Teachers or Healthy Families America Child Welfare Adaptation, including required training, assessments, and model affiliation.
Families served: Expectant parents and families with children ages 0–5, with eligibility extending through kindergarten entry where applicable.
Visits and support: At least monthly in-person visits, more often if required by the model, with every other visit in the home; include evening/weekend availability, parenting education, developmental referrals, prenatal and child health support, budgeting education, and transportation assistance.
Case management: Assessments, family plans within 90 days of referral, care coordination, transition planning, satisfaction surveys, six-month follow-up, secure records, and routine reporting.
Annual volume: Estimated children served: East Hawaii 150; West Hawaii 35; Kauai 35; Maui County 175; Greater Honolulu 150; Greater Leeward 170; Greater Windward/North Shore 40. Capacity must accommodate referrals without waitlists.
