What the job is
Service period: Anticipated December 2026 start through June 30, 2027.
Screening: Identify and screen at-risk clients using 2025 AASLD/IDSA HCV guidance, including people who use drugs, experience homelessness, have HIV, are pregnant, are Black or Indigenous or other people of color, or are transgender/non-binary.
Care navigation: Coordinate confirmatory testing, medical appointments, insurance enrollment, treatment initiation, prescription pickup, and medication adherence; provide nutritional supplements when needed with treatment.
Client support and reporting: Address treatment barriers, document care outcomes, manage consent-based information sharing, distribute supplied client incentives, and attend monthly care coordination meetings. Screening templates and reporting tools are supplied.
Insurance: General liability of $2 million per occurrence/aggregate; cyber liability of $1 million per occurrence/$2 million aggregate; auto liability of $500,000 when applicable; workers' compensation as required.
