QualificationsCurrent California dental license, graduation from an ADA-accredited endodontics program, current BLS certification, California DEA registration, and CURES access.
CoveragePlease identify your office locations and geographic coverage. Services are primarily performed at your office, with any on-site care arranged separately.
TermAnticipated start February 1, 2027, for 36 months, with a possible 12-month extension at the same rates; patient volume is not guaranteed.
