Service term: Three years.
Examinations: Primary and specialty eye care for youth residents by a Pennsylvania-licensed optometrist or ophthalmologist, including prescriptions as needed. Provider must bring examination equipment.
Glasses: Appropriate current-style frames; single, bifocal, or trifocal lenses; regular tint for glare; and diopters SPH#4, CYL2. Ship completed glasses to the facility's Health Services Department.
Repairs: Adjustments and minor repairs for broken frames, lost screws, and cracked lenses.
