Service period: January 1, 2027–December 31, 2028, supporting 20 reporting sites with anticipated growth to 30.
Data analysis: Develop secure collection and analysis processes for de-identified patient-level data from multiple EHR systems, standardized in Excel; include performance measure coding and data quality checks.
Reporting: Produce site-specific and statewide mid-year and year-end reports, due in March and September, and create, host, and maintain dashboards tracking enrollment, utilization, and services.
Training: Conduct at least five professional development meetings annually for 20–30 attendees; plan for four in-person meetings, typically three in the Augusta area and one in Orono, plus one virtual meeting.
Technical assistance: Support care standards, quality improvement, accurate data reporting, and sustainability, including monthly virtual planning meetings.
Pricing: Provide a fixed total for the two-year period, including staffing, travel, supplies, and dashboard hosting.
