What the job is
Services: Comprehensive eye exams, prescription eyeglasses meeting ANSI standards, lens replacement, and frame adjustments and repairs.
Volume: Minimum four juveniles per clinic unless otherwise requested; approximately 3–5 clinics per facility annually or 3–4 clinics monthly overall.
Pricing: Provide a daily clinic rate for four juveniles and exam and eyewear pricing for each additional juvenile. Include staffing, equipment, supplies, travel, and delivery.
Provider requirements: Kentucky-licensed optometrist with licensed backup coverage; provide full exam records and maintain HIPAA compliance.
Schedule: Anticipated service period October 20, 2026–June 30, 2028. Include weekday availability and ability to provide after-hours telephone consultation.
