What the job is
Scope: On-site assessments, staffing and resident care observations, staff/resident interviews, and tracking corrective actions, including medication safety, pressure injury prevention, and daily care.
Qualifications: Minimum three years of skilled nursing facility monitoring experience; SMQT-certified monitoring staff with nursing and/or dietary expertise.
Availability: As-needed coverage 24/7, including weekends and holidays; on-site response within 24 hours for urgent assignments and two weeks for routine assignments.
Reporting: Weekly PDF reports summarizing observations and corrective action progress, delivered securely with protected resident information safeguarded.
Pricing: Separate hourly rates for RN monitors, non-RN monitors, and generalist support staff. Itemize travel expenses separately within applicable per diem limits; travel time is not billable. No guaranteed workload or minimum service charges.
Term: One year, with four optional annual renewals; estimated start June 12, 2027. Rates must remain fixed for the first year.
