- StaffingSurveyors with required provider-specific training, SMQT certification for long-term care, a dedicated project manager, and an unrestricted registered nurse available to accompany survey teams as needed. Minimum three years of relevant survey experience.
- DeliverablesComplete survey evidence, citations, and investigation narratives within eight business days after survey exit; escalate immediate jeopardy findings within one hour.
- PricingProvide hourly rates by facility type for surveyors, life safety surveyors, RN surveyors, project manager/administrator, quality assurance analyst, and generalist. Identify travel and overnight expenses for statewide coverage.
- TermOne year, with four optional annual renewals; initial-year rates must remain firm. No minimum service volume is guaranteed.
