Staffing: Eight qualified Health Facility Surveillance Nurses, developmental disabilities surveyors, or associated clinical professionals available for immediate deployment across multiple locations.
Duration and volume: A 90-day period covering up to 512 workdays: 320 for Specialized Mental Health Rehabilitation Facility complaint surveys and 192 for Intermediate Care Facility for Individuals with Intellectual Disabilities surveys.
Services: Unannounced onsite monitoring, complaint and incident investigations, care observations, record reviews, and documentation of compliance findings at long-term care and specialized facilities.
Scheduling: Concurrent statewide teams, including evening, overnight, and weekend coverage as needed.
Reporting: Written reports within 24 hours of each visit and immediate notification of resident health or safety risks.
