Service term: Three years of services as needed, with an optional two-year extension at the same unit prices. Please provide pricing for each test type.
Testing services: Sample collection and laboratory analysis for random, reasonable suspicion, and post-incident testing, including computerized random selection.
Collection and analysis: Follow SAMHSA/CAP FUDT protocols, use split samples and documented chain of custody, forward specimens within 24 hours, and confirm initial positive results with a second test. Retain samples for one year.
Collection personnel: Background checks required for staff collecting or handling unsealed samples, subject to specified eligibility restrictions.
