Volume and termApproximately 2,000 exams annually, with no guaranteed minimum; three years plus two optional one-year extensions.
Clinic coverageAt least six clinics offering weekday hours covering 9:00 a.m.–8:00 p.m., plus weekend and holiday availability.
AccessAppointments and walk-ins, with maximum waits of 30 and 60 minutes, respectively.
Follow-upInclude follow-up exams for 30-day medical holds in the per-exam price, with clearance completed at the original clinic.
ReportingWeb-based exam status access, monthly activity and wait-time reports, and phone and email notification of failures or medical holds within three hours.
