Reporting: Results for employees and employer, secure electronic delivery and storage, and an annual report by December 15.
Service period: Anticipated December 23, 2026, through December 23, 2029.
Pricing: Provide rates per visit/exam by location, including travel and all service costs, plus rates for urgent, repeat, non-routine, and job reclassification examinations.
