What the job is
Quantity and size: Approximately 5,000 sets annually; 48 pages each, finished at 8.5" x 11". Exact ordered quantities required, with no overruns or underruns.
Paper: 32 pages of 40 lb. offset cover/instructions and 16 pages of 60 lb. white vellum-finish offset forms; 24 lb. white wove envelopes. All stock must contain at least 30% post-consumer recycled content.
Printing and finishing: Black ink on both sides from supplied artwork; saddle-wire binding and easily removable forms. Forms must work with BancTec S220-D and SDS scanners and retain a full 0.5" left margin.
Return materials: Securely inserted, removable envelopes and/or labels supporting two return addresses. Envelopes must hold up to eight sheets and meet regular first-class postal requirements; #10-compatible sizing preferred. Include black and PMS 712U pink printing, an 8.375" x 4.5" tear-off payment voucher, and approximately 3.5" x 1" white and pink address labels, or a suitable alternative. Please specify your proposed solution.
Packaging and pricing: Exactly 100 booklets per labeled carton. Quote per 1,000 for the initial 5,000 sets and additional 1,000-set orders, including printing, binding, wrapping, packaging, and delivery.
Ongoing work: Anticipated five-year arrangement starting December 1, 2026, with pricing firm for the first two years. Please indicate proposed annual increases for years three through five.
