What the job is
Sioux Falls staffing: One nurse team lead and five to seven nurse home visitors; one optional community health worker. All required positions must be quoted together.
Regional staffing: Up to ten nurse home visitors covering Pine Ridge/Southern Hills, Spearfish/Northern Hills, Winner/South Central Region, Pierre, Mitchell, Platte, Yankton/Vermillion, Watertown, Huron, and Brookings. Individual regional positions may be quoted.
Additional roles: Two program assistants/referral specialists, quoted together, and a separate continuous quality improvement specialist at approximately 0.2 FTE.
Qualifications: Nurses must hold current RN licenses; team leads require a BSN or higher, and home visitors require an associate nursing degree or higher. Required Nurse-Family Partnership, Bright Start, and HIPAA training must be completed.
Pricing: Include hiring, supervision, salaries, benefits, travel, supplies, technology, administrative support, and applicable office costs. Identify the positions and locations covered.
Term: Anticipated January 1, 2027 start for a five-year term.
