Timeline: Start on or before December 1, 2026, and complete services by June 30, 2027.
Program operations: Provide staffing, member recruitment and orientation, engagement, resource sharing, communications, and retention support for organizations employing CHWs.
Meetings and training: Provide at least monthly CHW meetings and semiannual trainings, including virtual and in-person peer learning and mentoring. Include continuing education coordination and content for general and perinatal CHWs.
Certification: Secure free enrollment for up to eight CHWs in a Maryland-accredited certification program.
Reporting: Present the program design and evaluation plan by January 6, 2027; track participation and outcomes and provide progress reports by March 15 and June 15, 2027.
Pricing: Include itemized personnel, training, certification, technology, travel, and other direct costs. Maximum total budget is $100,000; indirect costs are excluded.
