Term: Initial 18 months, with an anticipated July 1, 2027 start; price three optional one-year renewals separately.
Assessment and curriculum: Complete a needs assessment and standardized, accessible training materials within six months. Cover patient registry system use, standards of care, and program requirements.
Training: Deliver at least 12 provider sessions with 15 attendees each and 12 patient/caregiver/public sessions with 25 attendees each, plus training for all participating local health departments, currently eight.
Delivery and outreach: Support virtual, in-person, and hybrid formats; include scheduling, participant recruitment, engagement, and material updates.
Reporting: Track attendance, feedback, and effectiveness; submit activity reports within seven days and materials for review two weeks before training.
Pricing: Include all delivery costs and expenses. Each optional renewal includes 12 provider sessions, 12 patient/caregiver/public sessions, outreach, and reporting.
