• PackagingTear-, water-, and chemical-resistant plastic outer package, maximum 12 x 8 inches; self-sealing Tyvek-equivalent business reply return envelope, 9.5–11.25 inches long x 5–6 inches wide; and a self-sealing swab envelope, maximum 8 x 3.25 inches, attached to the form and containing a desiccant packet and orange biohazard label.
• Printing and labelsCustom 14 x 8.5-inch form on white 67 lb vellum bristol with two scores and one perforation; red permanent-adhesive shipping seal; four matching Code 128 barcodes unique to each kit and consecutively numbered; blue component/contact/reorder insert; and neon-orange change-notification insert in each shipping container. Printed proofs require approval before production.
• Supply schedulePrinted proofs within 30 days and initial stock available within 45 days of the initial order. Include ongoing stocking, distribution, and weekly electronic inventory reporting.
• PricingProvide unit pricing per complete kit based on the annual volume and 25-kit minimum orders, production and replenishment lead times, freight/postage costs for statewide delivery, and warranty details, including at least 12 months of defect coverage.
