Clients: Medi-Cal beneficiaries ages 5–21 and transitional age youth ages 16–25.
Staffing: Six full-time equivalent clinicians, each carrying at least 25 clients.
Services: Assessments, case management, intensive care coordination, and individual, family, and group therapy. Please indicate your ability to provide therapeutic behavioral services (TBS).
Facility and care: Medi-Cal-certified site within San Benito County, BH-CONNECT evidence-based practices, and services in clients’ primary languages.
Documentation: Electronic health records, CANS-certified staff, CANS/PSC-35 assessments, monthly reporting, and quarterly chart reviews.
Term and pricing: Three years with one possible one-year extension. Include hourly rates by clinician license type and service, plus itemized costs for bundled services.
