How to submit

Contract term
The contract has one 30-month base period and one 30-month option period. (Attachment 2, p. 61)

Pricing format
Proposal must be in two severable volumes: Volume I (Proposal Certifications and Price Schedule) and Volume II (Past Performance Information). (Attachment 1, p. 2)
List only one price per CLIN in each contract period. Ranges or split bids are nonconforming. Zero ($0.00) unit pricing will NOT be accepted. (Attachment 1, p. 3)

Return the completed schedule of prices for each period of performance (Attachment 2) in Volume I. (Attachment 1, p. 3)
Return the completed DLA Disposition Services Contractor Representative form, PWS Section 6.4. (Attachment 1, p. 3)
Return completed provision 52.209-7, Information Regarding Responsibility Matters (Attachment 4b). (Attachment 1, p. 3)
Return completed provision 52.229-11, Tax on Certain Foreign Procurements, Notice and Representation (Attachment 4b). (Attachment 1, p. 3)
Include offeror name, CAGE, SAM Unique ID, address, phone, and a statement of agreement with all solicitation SP450026R0015 terms. Acknowledge any amendments. (Attachment 1, p. 2)
Include statements of capability to work in Turkey and to perform hazardous waste removal and disposition, supported by Volume II past performance from the last 3 years. (Attachment 1, p. 3)
